When saving practitioner case notes to come back to later (before marking as complete), formatting is often completely changed. There are coding lines added to the beginning and end of the note (for example: <html> <head> <meta http-equiv="Content-Type" content="text/html; charset=utf-8"> </head> <body> ) All quotations "" are changed to " All paragraph formatting is changed to remove additional space between paragraphs making case note difficult to read.
